Restart Zotero, go into Word and take the sequence of actions that cause Word to crash, then submit a Report ID from Zotero. Submit it even if the dialog listing errors is blank.

If you remove the bibliography and all its duplicates from the document do you still see this behavior? If you are, you haven't removed all the duplicate bibliographies from your doc. If you do not, does this start again after reinserting it?
