@adamsmith, after doing some more work on this, it seems it's an issue with authentication in the browser. I do have access to the paywalled content. In Edge, I can authenticate and have Zotero import all the references from the issue page just fine…

Um, no, as a matter of fact. Troubleshooting mode looks fine, and now when I open Zotero in normal mode, it looks as expected too. Will remember to try that if something goes similarly wonky again in the future. Thanks so much! :-)

On issue #1, thanks so much, @dstillman! On issue #2, I'm on Windows, and the list appears on top of the citation dialog only when the dialog box is close enough to the bottom of the screen so that the list can't fully appear below the dialog.

@tim820, there was probably a better way to do it, but I realized I could export the itemAttachments table from zotero.sqlite.bak to a CSV and filter the CSV to identify the attachments saved with an S:\ path. :-)
