Try the latest beta. It looks like this could happen in Z8 if an OPTIONS request supports Basic Auth but other requests (e.g., PROPFIND) require Digest. (Maybe other situations too, but we can reproduce it with that.)

We distribute a desktop file in the tarball that specifies an icon. If you've run the set_launcher_icon script properly, you should see the icon in your launcher. Beyond that, you'd have to know how icons work in your desktop environment.
