I refreshed the library on my iPad so I could see all the notes and highlights on my laptop as well What do you mean by “refreshed”? What’s the zotero.org URL when you select the parent item of the attachment in question in the web library?

@daniel.r: Not yet, but subsequent attachments don’t get titled as “PDF”, so you shouldn’t be seeing that unless you’re merging items or dragging attachments from one item to another.

Can you provide a Debug ID from the Zotero Connector for reloading the page and trying to save? Current versions of the Zotero Connector and Zotero should download PDFs from the browser, not the app.
