Have you tried restarting your computer? Does it work in a new document after a restart?

Anything peculiar about this document? E.g. did someone else edit this document before your attempt now? Are there tracked changes in between the first and fifth citations?

We did push a fix for it, albeit not a fully tested one since it is unclear under what conditions the Startup folder gets placed in that location instead of the standard one. Are you having problems with the automatic plugin installer?

The current highest priority (HighWire) translator seems to indeed produce wrong metadata. You can right click on the Zotero Connector button and choose a different translator under "Save to Zotero" option. COinS seems to work well.

The progress bar is displayed by Zotero, not the browser, so you should look through the Zotero windows. If you cannot find a progress bar try restarting Zotero and your browser and try again.

We should probably just remove the bibliography from the document instead of display a non-descript error message in such situations, but the error is what it is - you cannot have a bibliography without having cited any items in the document.
