Find your Normal.dotm and move it away somewhere temporarily (like your desktop). Upon restarting Word a new one will be recreated. If it doesn't help, try creating a new Windows user account and see if the Zotero tab appears there.

You need to press Refresh in Google Docs in the Zotero menu after transfering the doc. Make sure you convert it to a Google Docs document first (don't edit as .docx).
