You need to reset your browser user profile, or create a new one, and/or try disabling other extensions that are running alongside Zotero Connector.

The file you sent us is saved as a .docx. Citing doesn't work in .docx files on LibreOffice, unless you use Bookmarks, but they will be prone to corruption, or may not work at all in tables.
