You have Zotero Word for Windows Integration disabled in Tools->Add-ons. Open Tools->Add-ons, click the Enable button next to it, and restart Zotero, and it should work.

I managed to get it by routing through my VPS. (Apparently the CDN is only broken for some regions?) Filed an issue, but item 1 there is most likely beyond our control, and item 2 may be difficult.

You have Zotero 4.0.17.1. This was fixed in Zotero 4.0.18, and the latest version of Zotero is 4.0.20.2. You can download the latest version of Zotero from http://zotero.org/download, but you should check that you have automatic updates enabled.
