I have a mac with Monterey (it was also a problem with Big Sur). I have the Zotfile plug in installed.

I am using Zotero 5.0.96.3 with the latest Zotfile and Skim (downloaded yesterday). When I extract annotations made in a PDF using skim, it extracts the highlighted text, but not the comments made.

For the new beta you no longer need Zotfile extension to extract annotations. Just right-click on an item and use "Add Note from Annotations". That will solve all your mentioned problems.

ZotFile doesn't know anything about Zotero annotations and will delete them if you use it on a file you annotated with the built-in PDF reader.
