    I've noticed that the subfolder structure doesn't respect or take into consideration the settings in the Renaming Rules -> Additional Settings in zotfile.

    Not currently. If you want expanded control over rename rules beyond %a, %y, and %t, you need to use Zotfile.

    I am not 100% sure what you mean by track, but Zotfile can extract PDF highlights (and I presume notes too, though not sure) and put them in Zotero as notes with a link to the original.

    You're aware that you can use ZotFile to turn attachments into links in any custom location you want to? That does have some downsides, but it should solve your biggest issue.
