If these items are not duplicated in the library, it should be possible to merge them automatically. For duplicate items, merge them in zotero, then refresh in word to merge. Note that group and library are two items.

UPDATE: I've noticed that the icon is being restored properly after double-clicking; and the issue doesn't reproduce consistently, so there may be some other process of judgment in between that is preventing the icon from being restored?
