@bwiernik thanks for the clarification regarding Microsoft Word. A little disappointing that LibreOffice Writer does not allow the same Multi-column layout compatibility for the Bibliography. However I will most likely stick to Writer.

Hmm, not sure what is the problem: Zotero 4.0.28.9 (2015-12-12) on Firefox 43.0 / Win7x64. Zotero is deactivated with message "could not be verified for usage in Firefox and was deactivated" (translated from German)

It indeed looks like it was a Zotero data directory problem. I have now re-created the data directory with Zotero Sync Server and then also in the "old Firefox" the import icon did show up again even having Cookie Monster active.
