@dstillman In my case it seems to have been necessary. I have placed Zotero.dotm in the active Startup folder but it was not automatically recognised by Word – hence the additional step. I probably have not explained it well.

This will happen in version 5.1 For now, you can add "DOI: ..." in the Extra field. This does the job for me.
