In Zotero, under "Cite" uncheck the box for "Include URLs for journal articles".

That's nothing to do with the style, but a data entry/import issue you are responsible for. Make sure your titles are stored in sentence case within Zotero. If they are not, right click >> sentence case.

Unfortunately that is not possible and you can find that requested regularly and the answer is always the same. Combining the chapters as .pdfs is generally what's recommended.
