As long as the Connector is able to access the cookies programatically it should be fine, but it might be the case that the preference prevents that access.

Are you on Zotero 4.0? That's most likely what's causing the error indicator to fail. Zotero 5.0 is just around the corner and that will be fixed there.

Are you saying you don't get an error message in the progress window on the PDF page after the save fails?

You'll need to get your hands on the Office installer. I believe you don't need to reenter the key, you just need to run the installer and modify your existing installation to include Visual Basic for Applications.

Does Adobe Acrobat install an extension in Chrome? See if the connector in Chrome still works if you disable the extension. Otherwise, we'd like to see a Report ID from the connector.

You should see a Zotero "Z" button by the preferences button in Firefox. If it is not there, see if you can add it by going into the preferences menu, clicking Customize and dragging it next to the preferences button.

Are you storing citations in this document as Fields or Bookmarks (can be checked in Document Preferences from the word plugin). If it is Fields, you should enable Field shading in Word Preferences -> View and see if the citations are shaded grey.

If your supervisor or someone else who you may have shared the doc with edited and saved it online, the result would be the same. If the codes are gone your only option is to work from an earlier backup.

Oh, I didn't realize your OSX was that old. We'll be discontinuing Zotero 4.0 soon and the minimum OS for Zotero 5.0 is MacOS 10.10. Something to keep in mind.

The chrome connector has been updated and requires a current version of Chrome (currently 58). We will not be supporting outdated versions of Chrome, and there are other good reasons to upgrade, including security vulnerability updates.

Ok, this is a Word, not Zotero issue. You may have opened the file twice in Word (or another user on your system has it open). Either way you should close all instances of Word (perhaps restart the computer) and reopen the file.
