ZotFile doesn't know anything about Zotero annotations and will delete them if you use it on a file you annotated with the built-in PDF reader.

They're just linked files rather than stored files. You would've had to go out of your way to create those (either manually or with ZotFile), so it's assumed that you understand the difference. Linked
