This is a standard Word feature, not a Zotero feature. If it's not working for you, you configured it wrong or there's a Word bug.

If you open Zotero.dotm manually do you see an "Add Note" button? If so, are you sure you are correctly placing it in Word's startup folder? If not, did you completely remove and reinstall Zotero by d
