I was able to extract all highlighted and comment bubble annotations from that PDF. While extracting, Zotfile shows a progress window in bottom-right, where under progress is marked by an incomplete circle. Zotero 5.0.10 with Zotfile 5.0.4

I could not reproduce this in 5.0.11. I tested with all three options in the search drop-down menu. Is your indexing complete? See Preferences > Search. Does the behavior changes based on your selection in the search drop-down menu?

What is the file extension for the RIS file? I've seen the file downloaded with .txt. If that's the case, change it to .ris and see if the file opens in Zotero.
