You don't say what's actually happening. If this is a large document, it's possible you're just not waiting long enough. Beyond that, we'd need more details. See Reporting Problems for the two things we always need to help.

It's working for me in STP. It disappeared briefly at one point but came back after an app restart, so the Preview might just be a little buggy. If it's not coming back for you, try deleting the app, rebooting, and reinstalling.

What @aborel says, but for fixing file imports in general, I would skip straight to resetting translators from the Advanced → Files and Folders section of the Zotero settings.

Reset your translators from the Advanced → Files and Folders section of the Zotero settings. If that doesn't help, we'd want to see a Debug ID for Zotero startup through reproducing this, using the "Restart with Logging Enabled…" option.

There's nothing to set up. There's a button in the center of the PDF reader toolbar to open up the annotation toolbar, which includes the ink annotation tool.

Make sure the citation dialog isn't just appearing behind your other windows. That's the most likely thing here. If you're sure it's not that, can you provide a Debug ID for reproducing this after restarting Zotero?
