It's working for me in STP. It disappeared briefly at one point but came back after an app restart, so the Preview might just be a little buggy. If it's not coming back for you, try deleting the app, rebooting, and reinstalling.

What @aborel says, but for fixing file imports in general, I would skip straight to resetting translators from the Advanced → Files and Folders section of the Zotero settings.

There's nothing to set up. There's a button in the center of the PDF reader toolbar to open up the annotation toolbar, which includes the ink annotation tool.
