Restart in Troubleshooting Mode (Help → "Restart in Troubleshooting Mode…”), which temporarily disables all plugins. If that fixes it, you'll need to identify the plugin that's causing it and then report the problem to the plugin's developer.

Make sure the citation dialog isn't just appearing behind your other windows. That's the most likely thing here. If you're sure it's not that, can you provide a Debug ID for reproducing this after restarting Zotero?

Not as easy as letters, but you can use Alt/Option + a number to switch between tools, and the one associated with the color (i.e., Option/Alt-8 currently for PDFs) will cycle through the colors.
