This sounds like what was fixed in Zotero 9.0.4 a few days ago. Make sure you’re still seeing this in 9.0.4, and if so provide a Report ID after reproducing it.

That means an instance of the second one is still in the document. You’ll need to delete it in the document. The linked page explains how you can find it if you’re having trouble. (We have a better method for doing so coming.)
