Don't do this manually, that's why we have Zotero and csl styles. We have hundreds of numeric styles on the repository. What journal are you publishing in? https://www.zotero.org/styles?format=numeric&dependent=0

That would indicate your data is not entered correctly in Zotero. If yes, you might have somehow an item in your document that might not be in your database anymore.

You should always update any software, not only for bug fixes, but for security issues also. And in this case, Zotero has nothing to do with that. It's about the citation style you're using.
