Where do you put your annotations? In the note field? You could create a custom CSL style that contains whatever is in the note field. Easy peasy.

Before pointing fingers, did you check you're actually using APA and not ASA as in American Sociological Association? It would be weird if one of the most widely used styles on the repository is off, no?

You can suppress the authors. When in word citing something click on the name bubble to find some more options (page etc.). There you can also suppress the authors.

If this is for a specific journal, which is it? I'd rather create a custom style for the repository, so many more people can use the style and not just help one individual. :)

Turn on field codes (see step 9) and then search your document for those stray references. It is possible that there is some remnants of code left that cause the ghost entries in the bibliography.
