Can you link to the guidelines and an open-access paper that shows this requirement and say what exactly is wrong? Then we can look at this quicker.

Apart from that, if there an error with a style, we'd rather you tell us so we fix it for all the thousands of users of Zotero (and other CSL compatible softwares).

Take that APA style, replace the "citation" section with the one from the Vancouver style. That'll be the easiest.

Zotero is free. You can download it and play around. Specifically Zotero's "Tag" system will interest you as well as the "Collections" one can create.

Merge them in your Zotero database. Then refresh your citations in the document. That should do the trick.

Basically you can just switch the citation style to a note style and it'll do that for you. Assuming you've used Zotero to insert the citations in the first place.

The specific citation style does this on purpose. It replaces an author with the dashes to show that it's by the same author. Choose a different style or edit the given style.

You're mixing the two "pages". Ones a locators, to locate a specific section in a book/chapter ("find the statement on page 33"), the other is the Bibliographic information where the actually find the item. Book chapters have pages as that helps you…
