It's likely that this is caused by other extensions or security software. Try installing the Zotero Connector into a new Chrome profile to see if it works there.

Can you open developer tools (in Chrome: Tools -> Developer tools) on a Google Docs page, switch to the console tab, copy the contents, paste them somewhere and post a link here?

You can remove items from the bibliography by pressing Add/Edit Bibliography after inserting it. It will open up a dialog that will allow you to remove entries.

Seems like you have a corrupt translator in your Zotero data folder. In Zotero Preferences -> Advanced -> Files and Folders press "Reset Translators..." and restart Zotero.
