This is a variation of an existing LibreOffice issue. The horizontal line is just a paragraphy style in LibreOffice with a Next Style property set to Text Body.

Are these institutional computers or are you troubleshooting for a private computer? Could some group policy be overriding this? Can you create a new Windows user account on this computer and see if you get the Zotero tab there?

@ccm5u can you reproduce this in a new document? If not, then it's most likely caused by some item that you have cited having an URL field that contains a quote " character. You can fix it by replacing it with a %22 in Zotero.
