This just sounds like a Word, not Zotero, issue. A general rule of thumb for these sorts of errors is to try restarting the computer and seeing if it works better.

Could you be more specific about "the first item"? Is this within a single footnote citation with multiple items cited, in which case, if there is only one cited item, does it not occur? Is it the first footnote in the document? On the page?

With "item" I meant Zotero items. Do only specific Zotero items cause this or is it any one? What happens when you insert regular footnotes in Word in between the Zotero-created footnotes. Same spacing issue or do those look right?

Right. Zotero 5.0.43 does not yet include the citeproc version which fixes this issue. @Phenox you should either install Zotero Beta or the Propachi plugin into Zotero 5.0.43 to fix the bug.

Sorry, seems like we introduced a bug that makes the initial citation after restarting Zotero very slow in a recent update. We will fix this soon, but in the meantime you may download the earlier 5.0.41 version that does not have the bug.
