I am not seeing this in the document that you have sent us. It has 3 citations of the same item and only the very first one is a full citation.

What version of Java do you have set up in LibreOffice (check under Options -> Advanced)?

Something is wrong with your proxy schema. Go into Zotero Connector Preferences -> Proxies and remove all configured proxies, then restart your browser and try opening a proxied website again. The connector will re-add an appropriate entry.

This is very likely Java related, so that would be the first thing to troubleshoot. What kind of Java do you have set up with LO right now (check under Preferences -> Advanced)?

This is how bookmark encoding in Word works. If you want to use bookmarks as intended by these word processors it makes it difficult, but unfortunately this is the only way for us to support bookmark citations across the Word and LibreOffice

So whatever is doing this is not related to Zotero. You will have to figure out what is causing the breakage/modification of your Normal.dotm. A new user account on the same computer might not have this problem.

Zotero has not supported OpenOffice for quite a while now, since the software has been pretty much abandoned by its developers. You should install LibreOffice, which is the spiritual successor of OpenOffice and still actively maintained.

Do you have any other addons for Word? If you close all open Word documents and reopen them do you get the tab for any of them?

Resetting the template or disabling all macros with notification worked?

If you have chosen “Disable all macros with notification” for Word macros and don't get a notification then it's worth to try resetting the default word layout template by renaming it to Normal.dotm.bak and restarting Word.
