It hapenned to me today with beta.21. They didn't appear in the left pane when I synced. I restarted the app and they appeared in the PDF and in the left pane as well.

I use the Zotero Annotations Count Plugin, which adds a column showing how many annotations your items have. https://github.com/Dominic-DallOsto/zotero-annotations-count For me it is ok, I do not know if it can help you.
