I just tried using the style in Word for Windows, and here it works as required. So the problem resides in LibreOffice and not in the csl code. Thanks for taking the time to answer.

Yes, I sent the current pull request about 12 hours ago. I kept the font-style="italics" bit in, even though it doesn't seem to work, since it does no harm either.

I considered the "italics in word bibliographies" problem, so I tried to put a bit of non-italicized text in before the title. All of the text came out plain, so it doesn't seem like that is the issue.

Thanks for clearing that up for me adamsmith. Just to be sure, that I understand you correctly: If you use a specific passage from say a book, you don't note where in book the information is located? It seems to me to be rather harsh on the reader.

Strange. I'm using Zotero for Firefox 4.0.29.16, and I just reinstalled APA 6th edition, and I still get the same result, i.e. only initials instead of full first names in the bibliography.

That's true. In danish dictionaries you will commonly find words beginning with "aa" on the last pages. I had no idea that the sort mechanism will replace spaces with characters, otherwise I might have guessed that the special treatment of "aa"…

I don't think you can change the formatting of a dependent style. You would have to change the parent style, or rather: make a new independent style, that is a copy of elsevier-vancouver, and add the et-al-min attribute to the citation element.
