Running 5.0.97-beta.43+c5d89f6d0 (with connector beta, zotfile,mdnotes,better biblatex,zutilo all on latest).

The Zotero beta includes a new PDF reader that can add annotations to notes itself. You don't need to use ZotFile for that if you're using the beta, and ZotFile isn't currently maintained.

@nathan.hopson: Yes, ZotFile's "Use actual article/book chapter page" is using the "Pages" field, so these offsets won't work for books. (You can find more technical details here.)
