It's from embedded attachment notes. Most likely you were using the ZotFile plugin to automatically populate those notes with a table of contents.

Now I just export using zotfile and then open through gdrive. All scribbles and annotations are preserved when the item is reuploaded from the tablet folder to its respective places.
