It's in the works. For the technically curious: CSL has been extended to allow for formatting of et al., and a new javascript CSL processor which will support this new feature is being build to replace the existing one used by Zotero.

A: The DOI field needs to be added. In both cases it is lower case “doi:XXXXX’ and comes at the end of the bibliography entry. The DOI-field is already added in the current AGU style for journal articles.

You have to check the option "Be lax about HTTP Content-Type" on the validator.nu website. (the website seems to be designed to validate websites more than standalone files like CSL styles, so it exhibits some quirks like this one)

Do you still have an item in your collection that you saved from the Lancet website? What is filled in in the Repository field? I can't find any mention of an existing translator for the Lancet at all either in the code or on the forums.
