Entering the DOI at http://dx.doi.org/ resolves to the article, so the DOI is correct (and the fact that PubMed doesn't store a DOI doesn't have anything to do with Zotero, so this discussion can probably be closed at this point).

Can you give us the PubMed ID of the paper in question? It's likely that PubMed just doesn't (properly) store the DOI for the paper.

For video recording, I think that the fugue icons "camcorder", "film", "monitor-image" or "television-image" could all work. I think I like "camcorder" best.

Did you change the term to "et al.", or "et-al"? You should use the latter. (term names in the Citation Style Language aren't necessarily the same as the US English term text)

Are you using Zotero for Firefox or Standalone? Which version? Can you reproduce the problem by creating another "report" item? If you are using Zotero for Firefox, try disabling all other Firefox Addons.
