Sorry about that. The built-in PDF reader has been completely rewritten in Zotero 7, currently in beta, so whatever this was, it's unlikely to be reproducible in that. If you see it again in Zotero 7, let us know.

Did you by any chance delete a PDF page outside of Zotero? E.g., a cover page? The annotations are just off by a page. Can you share a screenshot that includes the annotations sidebar on your iPad?

Can you provide a Report ID to start? What exact macOS and Word versions? Can you reproduce this in a new, empty document, or only in an existing document? If the latter, approximately how many citations, and how long have you waited?

The iOS app syncs automatically. You can do a manual sync by pulling down on the items list, but that should almost never be necessary.

Ah, OK, when you copy files between libraries, we copy the entire attachment folder, including the supplementary files (.zotero-ft-cache, etc.), and I guess we preserve the date on those. We'll investigate the upload issue.

Though, actually, we may have a fix for those sites — IEEE Xplore, very likely — already submitted to Google. Basically, sites where the page is built dynamically after it's first loaded may not work properly in the current version for Chrome.

Are the date and time on your computer wrong? That file appears to have been created 4 days before the item was added to Zotero. Can you provide a Debug ID for opening that file locally?
