@ABVJ: We'd need to see the PDF in question, either linked here or sent to support@zotero.org with a link to this thread. There's obviously no general problem with displaying images in PDFs in Zotero.

@Kimiora: Could you ask someone to contact us at support@zotero.org about this "security issue"? This is the first we've heard of this, and this is likely some sort of misunderstanding. There's no reason you should need to stop using the desktop app.

What macOS version? Restart your computer and try again. If it's still not working, reinstall Zotero from the download page.

Note that you don't have to get there through the Developer tab. The linked instructions include a step for opening that window via File → Options → Add-ins → "Word Add-ins" in drop-down → Manage.

You're saying you have an empty library with the welcome message when you open Zotero? You may have accidentally deleted your Zotero database. See Locating Missing Zotero Data.

But if it doesn’t happen in Troubleshooting Mode, and it happens again when you restart normally, the problem is one of the plugins you have installed. Troubleshooting Mode disables all plugins.
