If the local file modification date changes, Zotero should re-upload the file if you click the Sync button. You can also of course just add the PDF as a new attachment.

Are you running Zotero 7? Can you provide a Debug ID from the Zotero Connector for reloading the page and trying to save?

Are you still seeing this? Does it happen after restarting Zotero? If so, can you provide a Debug ID (different from a Report ID) for a sync that produces it? After that, try emptying the trash.

They're just item type icons. You can see the item type for the item in the item pane after selecting the item. Those are the Journal Article and Magazine Article icons.

And to be clear, if you're using Zotero on shared computers, you'd want to make sure to always go to the Sync settings and unlink your account and remove local data when you stop using the computer.
