You’d have to troubleshoot that yourself on your server using the server logs. This isn’t a problem in Zotero.

@daniel.r: Not yet, but subsequent attachments don’t get titled as “PDF”, so you shouldn’t be seeing that unless you’re merging items or dragging attachments from one item to another.
