Use this to make your style indented properly: http://formatter.citationstyles.org/ and I would keep ID and self-link as: ...vancouver-brackets-with-pmid (not sure if it allows spaces, but they should match anyway)

It's funny that they say this in the guidelines: "The typography to be used is that found in recent issues of the journal; it shall be used with scrupulous attention to accuracy." But then their articles have loads of discrepancies....

You should definetly have different filename, title, self link and ID for your style. (from my own experience with Mendeley)
