You need to also change the ID and self link in the top of the .csl, otherwise Zotero doesn't know the difference.

It's not a problem with the .csl. As you can see it works fine in the style viewer you linked to. Some of the Zotero gurus will need to tell you how to get this working.

Cited in this case means the date you saved the citation and not the date you put the citation in your Word document. The wording cited/accessed means the same here and can be changed very easily.

can you check in Zoteroth at the book is saved as a book and not as webpage. You can also try adding the book via its ISBN.
