We test for volume and page as that's a pretty sure giveaway that an article has been published. If not you'd print the DOI. As pseudocode: if not=volume AND page print DOI

Without checking, this is probably a dependent style. In line 6 or so is a link to a style name with the attribute "independent-parent". That's the style you want to edit. Note though a space before the brackets, you can just type in your text. I d…

You want to look at the "Bibliography" style in Word directly. I think it's called Styles and Formatting. Another option is to switch citation styles to a completely different one and try in a new document.

You can ignore italics. Small caps use caps. The rest doesn't matter.
