In the bibliography it will only list one item, even if you referenced it several times with several different pages throughout the text.

They do not correspond to anything in CSL. They are added in the document itself. What output do you want to achieve? Share your code on pastebin.com or similar if you wish.

OK. I cannot find a publicly available .pdf of a recent journal article in order to confirm this request and then put in a fix for the style. If you have a link to a publicly available article, that'd be great. I searched for 15min.
