Option 1. Even though it might seem strange, this could still be expected since all citations in notes are stored in this manner. Have you ever needed to have a plain text citation in a note?

This is weird and I haven't seen this before. The question is whether all Windows users experience this or this is something specific with your system. Normaly it works as you would expect from other text editors.

Also, it seems that there is no way to produce a simple citation to an item in the PDF viewer to a Zotero note? Maybe a drag and drop of the tab title to the note could do the job nicely? For the start, I think, a citation copied with 'Edit' → 'Copy…

@nilssssssss Could you explain what do you mean with "it gets tricky to select a box to drag it"? In future there will be other ways to change font size.

Well, this is intentional but we might want to reconsider that. At least if single annotation is being dragged. Although this steps shouldn't be necessary if you just want to copy a citation for the current page. Instead, maybe Edit → Copy Citation…

You can filter annotations by color in the annotations sidebar. To select all filtered annotations, use Cmd/Ctrl + A. Then, right-click and choose 'Add to Note' from the menu. Make sure a note is open in the right-hand pane before doing this.

I reported the issue to PDF.js, and they seem to be aware of it, although they haven't made any progress yet. Let's wait until it's fixed.

It sounds like a problem with this specific PDF file. Could you test if Firefox PDF viewer (on which Zotero PDF reader is based) can properly display all pages?

Do you mean you can't copy selected text from Zotero reader using context menu? If so, does it work if you copy from a note?
