Is there a reason you can't upgrade to Firefox 3.5? Doing so (and installing PythonExt 2.6.2) would probably take care of this issue.

Re: the citation issue, do you have other Firefox extensions installed? The MinimizeToTray for FF 3.5 extension in particular is known to cause problems.

Unfortunately, I can't reproduce the error. My guess is that it's a Word-related bug with mixing right to left and left to right languages, but that's just a guess. Do you have all of the latest Word-related updates installed?

Try quitting Firefox, opening /Applications/Utilities/Terminal, and typing sudo rm ~/.zoteroIntegrationPipe followed by a return. When prompted, enter your OS X password. Then restart Firefox.

The error message indicates that something has gone wrong internally with Microsoft Word, and nothing beyond that. I haven't received your document (perhaps it was lost in transit?), so I have no idea what that might be.
