Is Word open when you're opening Firefox? Zotero doesn't seem to be able to remove the old Word plug-in. If it isn't, try removing the old plug-in manual (it will be in the directory described here).

I agree; this shouldn't be happening. I managed to reproduce it a couple of times in Firefox 3.6 on Windows, but then it disappeared apparently out of the blue. If I can get it to happen again, I'll try to figure out what's behind it.

I think this was caused by an invalid regexp in the NYTimes translator. You can try resetting translators in Zotero Preferences->Advanced. If that doesn't work, delete the translators subdirectory in your Zotero data directory.
