I'm not sure how much I can help with this because it really sounds like a LibreOffice bug, but do you have a JRE installed and seelcted in the Java section of the LibreOffice preferences?

I'm still looking at this, but my hunch is that it has something to do with the fact that the document appears to have both footnotes and endnotes. I'll fix this, but for now you might try converting one to the other by following these steps.

1) Does this happen on any website? 2) Assuming you are using Chrome, do you have other extensions installed? Have you tried disabling them? If you can isolate which is causing the problem, we can take a closer look.

That's definitely not supposed to happen. Do you have any non-Zotero extensions or themes installed? If so, have you tried disabling them? Otherwise, can you generate a report ID after re-triggering the issue?

Can you open other PDFs in Firefox? Try this one. If you can't, this is a Firefox issue, not a Zotero issue. You might also try right/control-clicking the PDF and selecting "Open in External Viewer."
