You don't say what operating system you're running on, but if you're running Linux, it might be this. In any case, upgrade Firefox; Firefox 11 does not receive security updates.

If there's nothing in the install directory, it sounds like someone or something deleted Zotero.dot from the extension directory. Try reinstalling the extension from zotero.org, and then see if you can reinstall the add-in.

If you have Zotero Standalone open, click "Update now" in the Zotero preferences. Then restart Chrome (or clear the cache and disable/re-enable the extension).
