Do you have other add-ons installed into Word? Have you tried disabling them? Can you create a new Windows user account on your computer and check if inserting there with a superscript style works fine?

While Zotero addons shouldn't affect feed processing, it is not out of the question that they do. If you disable all Zotero addons and restart Zotero can you then add the feed?

It's possible that you had multiple copies of the item in your Zotero library and later removed one of the duplicates that is now cited in your document. Remove all occurrences of the cited item in the document and reinsert.

Yes, this is not supported. If anyone edits and saves the document in some other editor than Word then citations will get stripped out.
