How did you get this version number? Can you tell Word version under File -> Account -> About Word? Did you try creating and testing other notes with images? Do none of them insert correctly?

Find your Normal.dotm file and try to move it somewhere else (like your desktop), then restart Word. This will regenerate your Word layout settings if you or someone/something else has modified them.

What's the full version number of Word that you are using? What citation style did you choose in Zotero? Presumably this is happening in a new document? If not, can you reproduce it in a new document?
