The Zotero in your browser is the Zotero Connector used to save information into your Zotero library while you browse the web. You should not delete it, it is a complementary extension to the main Zotero app.

This is controlled by the Bibliography style in Word. You have to modify the style, that way your formatting will persist.

Try running in Troubleshooting Mode (Help -> Restart in Troubleshooting Mode) and see if it still occurs. If it does not, one of your extensions is causing it.

You will have to follow the troubleshooting steps above. In the past some users reported issues with Tables of Content and Indexes. It might be worth a try removing it if you have one.

This isn't a general issue. What is the full update number of Windows that you are running? By "refused to start", what do you mean? Did you get an error, or nothing happened? Do you run any proprietary security software?

Press Escape in the yellow dialog to dismiss it. If you simply switch focus to Word, the dialog just goes to the background, and another click on Add/Edit Citation simply refocuses the already open yellow dialog.

Does it work if you restart Zotero in Troubleshooting Mode (Help -> Restart in Troubleshooting Mode)?

Your Debug ID doesn't seem to include the initial button press of Add/Edit Citation, but either way, it's very possible that the Zotero Style plugin is somehow breaking this, so you should disable it, restart Zotero and see if it works then.

Please describe in detail what happened at each step of the troubleshooting instructions. The final step by definition either leads to an empty document or fixing of the problem, so it can't "not work".

You might need to restart Zotero in between attempts to troubleshoot this. Does it work in a new document? If not, could you restart Zotero, then generate a Debug ID for an attempt to add a citation?
