You should accept or reject any tracked changes before attempting to work with Zotero as that is likely to create issues. If any one of your collaborators opened the document in a different text editor your citations may have become unlinked.

If you put word cursor in those citations that get unlinked and press Add/Edit Citation in the Zotero tab do you get a citation dialog that is empty, or with those citations?

Does it work if you disable the grammarly extension? As far as I am aware it is quite invasive in terms of interacting with many elements of Google Docs editor and is likely breaking the Zotero plugin.
