Can you explain step by step, after restarting Zotero, what you do in the Zotero interface, to trigger the issue, what you expect to happen at that step, and what happens instead? Also submit a Report ID.

See https://www.zotero.org/support/kb/citations_not_updating You will have to manually reinsert those citations which are unlinked. Your workflow of exporting and importing the items unlinks them from items cited in Word, so you should not do that.
